4⃣ If your family, roommate, or friend becomes a close contact, please refer to the flowchart 4

If your roommate or friend has symptoms:
 
Please stay at home until the PCR test result of your roommate or friend is known.
  If it is negative, you can come to school/work.
  If it is positive, please refer to the flowchart (3) of what to do if you become a close contact and take
  necessary procedures.

What to do after you are requested to stay at home
  1. Stay at home and call the Iwate University health care center (019-621-6074)
    (Office hours: 9:00-17:00, on holidays and outside of business hours: call the center on the
    earliest week day)
  2. Students: Report to the instructors in charge of your classes (via WebClass) that you have been a
    close contact.
    Faculty and staff: Report to your department that you have been a close contact.
  3. Describe the behavior history and health condition from one week prior to when you were
    asked to stay at home in "Movement history and physical condition report (students)
    (faculty and staff)". It might be requested to submit it later.
  4. Report your health conditions using app, "健康日記(Kenkou Nikki)" every day until you are
    allowed to come back to school.
  5. Avoid unnecessarily going out and minimize the number of times you go shopping.

If symptoms appear during the stay-at-home period
  Please be sure to inform your doctor or medical institution that you have been staying at home
  and then go to see a doctor. After that, please contact the Iwate University Health Care Center.
  Also, please submit the "Movement history and physical condition report (students)
  (faculty and staff)".
  ・Please set the file name as "Your Name".
  ・Please change "(at)" to "@" and send the e-mail address to "kenkou(at)iwate-u.ac.jp".

If your roommate or friend has no symptoms
  ●If you have symptoms (even mildly)
   Stay at home and refer to the Flowchart (1) for what to do if you have cold symptoms or suspect
   infections

  ●If you have no symptoms
   Students/faculty/staff can come to school and go to work.
   For one week after the end of the stay-at-home period, body temperature and other health
   conditions must be monitored, and eating and drinking with others, part-time jobs, and club
   activities will be prohibited.
   Please use the app, "Kenkou Nikki" to monitor your health.
